Home sleep apnea testing (HSAT) and in-lab polysomnography (PSG) both diagnose sleep apnea, but they work differently, and each comes with pros and cons for how you operate, what you can bill for, and which patients you can serve. Here's a closer look at each, to help you decide which fits your business need the most. 

What Is Home Sleep Apnea Testing (HSAT)?

HSAT is a portable diagnostic test patients take at home to screen for obstructive sleep apnea. Most devices track airflow, breathing effort, oxygen saturation, heart rate, and often snoring and body position overnight, then a licensed sleep physician reviews the data to calculate the patient's Apnea-Hypopnea Index (AHI), the primary number used to diagnose obstructive sleep apnea (OSA).

What Is In-Lab Polysomnography (PSG)?

PSG is the traditional overnight sleep study, conducted in a lab or hospital with a technician monitoring the patient in real time. In addition to breathing and oxygen levels, the in-lab sleep test also captures brain activity, eye movement, and muscle activity. This allows it to detect sleep disorders beyond OSA that require more specialized equipment.

Pros and Cons of HSAT

For patients showing signs of OSA, home testing can accurately identify the condition roughly 90% of the time. Alternatively, most at-home devices are not able to diagnose more severe sleep disorders and an in-lab is needed. Let’s break down the pros and cons.

Cons

  • Best for uncomplicated OSA, with some patients requiring in-lab testing instead. 

  • Not appropriate for every patient, including some with significant cardiopulmonary or neuromuscular disease, chronic opioid use, prior stroke, or other conditions where comprehensive monitoring is recommended

  • May underestimate OSA severity since total recording time rather than confirmed sleep time is used to calculate results 

For many organizations, HSAT serves as the primary diagnostic pathway, with referral relationships supporting the smaller subset of patients who need comprehensive in-lab evaluation. Sleep testing should always be prescribed and reviewed by a licensed sleep physician to determine the best next steps.

Pros and Cons of In-Lab Sleep Studies

Home sleep apnea testing and in-lab polysomnography aren't competing technologies so much as complementary ones. HSAT is designed for uncomplicated cases of suspected obstructive sleep apnea, while PSG provides the broader diagnostic capabilities needed when symptoms suggest a more complex sleep disorder or when home testing doesn't provide a clear answer.

  • Comprehensive physiologic monitoring, including brain activity, eye movements, muscle activity, breathing, oxygen levels, and other sleep metrics 

  • Broader diagnostic capability for central sleep apnea, narcolepsy, periodic limb movement disorder, and other sleep disorders beyond OSA 

  • Real-time technologist monitoring, allowing data quality issues or sensor problems to be addressed during the study

Cons

  • Higher operating costs than HSAT because of specialized facilities, equipment, and staffing

  • Greater operational complexity, requiring dedicated sleep labs and trained sleep technologists

  • Longer wait times in many markets because of limited laboratory capacity

  • Slower path to diagnosis compared with home testing

  • Less natural sleep environment, where the first-night effect may influence sleep quality

These operational considerations extend beyond patient convenience. Limited sleep lab capacity and workforce shortages can also affect access and program scalability. 

The sleep technologist shortage is real and industry-recognized. In 2024, the AASM formed a national committee to address it, and most program directors expect demand to keep outpacing new technologists entering the field.

Home-based sleep testing can help triage sleep disorders into two pathways, leaving in-lab visits open to those needing more specialist care for complex sleep disorders.

Rather than building both capabilities in-house, many organizations use HSAT as their primary testing modality and establish referral relationships with accredited sleep labs for the smaller number of patients who require comprehensive evaluation.
